Pick all the edges of the inner collar and extrude twice:

 
   
 

The first extrusion will remain where it is, the second one you lower so it gives the collar edge some thickness:

 
   
  You can also see in the rendering how the inside of the collar has been dropped:  
   
 

See how important it is to give the edges thickness, otherwise the shirt looks like it's painted on kinda ... at least that's my preference, to give it thickness. Should also give bottom of the shirt a thickness, the same way. I'll leave that to you. So ... what now? Oh, you could move the vertices a little around the V of the collar to make it a little less smooth ... I dunno, its up to you. Or when you Meshsmooth you could leave the collar V outta the process so it looks a little sharper....

I may texture it ... and show it textured before I move on. I don't think I'll write a tutorial about that as it would require a whole intro to a texture mapping program!
