  SLEEVES--Lengthening, Scaling
And extrude again. Three times total, so you get the sleeve reaching the elbow ... you can go further of course, and cover the whole arm, for long sleeves. Adjust the width of the sleeve down its length by selecting edges cross-wise and scaling in the view from above the hole (the right view in this setup--see third picture down). Select edges by grabbing the area best you can, and then remove the edges you don't want by deselecting while holding ALT.

Now we got the ones we want, we scale outward in the view shown below, along XY , starting with the set at where the arm joins the body ... you know you got em all if edge count 14 (each counts as 2 as for some reason the count doubles per edge when u got polys on either side); edges count at end of arm 7. You can move individual vertices for finer tuning. Or to make the sleeve a little less perfect.

After scaling the four sets of edges:

And below, shirt after MESHSMOOTH. You can MESHSMOOTH at any time to see where you are going, and then remove the MS modifier when you are ready to continue ... Note if you pick a tighter mesh to drop you get better results (ie, more polys), ... it's just more work cuz you got more edges. And more extrusions. Remember I picked very simplest 8x8 planes to start. Could do with another arm!! So repeat on that side what you did on other side.
